If you would like to refer a client for services, please submit the following to SCSI:
Please Fax: 860-443-8940,  Attn: Sarah Clark

 

  • Referral Form (please complete all fields)
  • Release of Information
  • Discharge paperwork/med list if available

Copies of these forms are below in PDF format for your convenience. Clients who are self-referring can either be screened over the telephone or may walk-in during regular business hours. Clients are no longer required to attend the Tuesday “Open Access” Orientation Clinic. Please feel free to share the flyer and/or the referral packet with your co-workers, and give the flyer out to any potential clients!

We hope this will help reduce any barriers to care due to transportation! Once screened and if appropriate for services, we will schedule an intake for the client at the appropriate location: Norwich or New London.
